Or, most commonly, a tearing sore throat and fever that make the thought of working at the computer seem as realistic as running a marathon with tonsillitis. It\u2019s at moments like these that many of us face a dilemma: do I really have to take the tram, go to the clinic, and prove that I\u2019m sick? Do I really need to infect others in the doctor\u2019s waiting line just to get an online medical sick leave?<\/p>\n

Just a few years ago, the answer was yes. Today it\u2019s absolutely not. Modern medicine, despite appearances, is not limited to operating rooms and nanotechnology. Its real revolution often happens quietly in our homes, when we click to schedule an online consultation and, dressed in sweatpants with a heating pad on our stomachs, talk to a doctor through the camera. Online medical sick leave, meaning L4 issued remotely, is nowadays one of those changes that really make life easier. But how does it work? Can everyone use it? Do you have to show yourself on camera? And just as importantly: what does communication with the employer and ZUS look like when we no longer receive a physical form?<\/p>\n
E-sick leave, officially known as e-ZLA, is an electronic document issued by a doctor, which is sent directly to the ZUS system and the employer. You don\u2019t have to pick it up in person, deliver, fax, or mail it. Moreover, you don\u2019t even need to get out of bed if you choose an online consultation. Sounds simple? Because it is. But it\u2019s worth knowing that despite all this technological convenience, specific procedures and criteria still apply that determine whether the sick leave will be issued.<\/p>\n

The basis is, of course, a conversation with the doctor. It can be a phone or video consultation, or a medical form filled out on an online platform. In each of these forms, the doctor analyzes reported symptoms and the patient\u2019s health status and, if deemed appropriate, issues the e-sick leave. There is no automation or sick leave on demand here. Every decision is individual and made based on professional judgment. Of course, some cases are more obvious: flu, covid, upper respiratory tract infections. But there are also more complex cases. These can be neurological problems, back pain, or anxiety conditions that require broader diagnostics, even if the contact takes place remotely.<\/p>\n

When the doctor issues e-sick leave, the document goes directly to the PUE ZUS system, the Electronic Services Platform of the Social Insurance Institution. If the employer has access to the system, they see the sick leave almost immediately. The patient doesn\u2019t need to do anything other than rest, of course. This is a huge convenience not only for the sick but also for people who cannot afford to stand in line or infect coworkers just to complete a formality.<\/p>\n
Anyone who pays a sickness insurance contribution can use e-sick leave. This includes both salaried employees and self-employed individuals running a business, provided they have registered for voluntary sickness insurance. Yes, a sole proprietorship can also have L4, but only if it was previously registered with ZUS under the appropriate code and the contribution is regularly paid.<\/p>\n

Importantly, the right to sickness benefits arises after a waiting period, which for the self-employed is 90 days. That means if an entrepreneur has just registered for sickness insurance and gets sick after one week, they will not receive benefits, although e-sick leave may still be issued. Salaried employees have it easier in this regard, as they are covered by compulsory insurance and acquire the right to benefits after 30 days of employment.<\/p>\n

And what about the procedure for obtaining e-ZLA online? In practice, it\u2019s very intuitive. We choose a telemedicine platform, and since you\u2019re here, you can use ours. We also offer medical consultations with e-sick leave. Then we fill out a short form indicating symptoms and duration of ailments, and then we have a conversation with the doctor. If the doctor deems that the health condition requires rest, they issue the L4 starting from a specific day, most often today. Despite digital convenience, sick leave is still an official document that carries certain obligations. Above all, sick leave means we are unfit for work. Interestingly, the doctor issuing e-ZLA can also pass information to ZUS about the necessity of specialist treatment, rehabilitation, or further consultations. In some cases, especially if the sick leave lasts longer than 30 days, ZUS may summon the patient for a control examination. It\u2019s worth being prepared for this and having medical documentation confirming the incapacity for work.<\/p>\n
E-sick leave is not a privilege or a novelty for the lazy. It is a response to a reality where the pace of life, availability of doctors, and the need for flexibility are more important than the traditional ritual of paper L4. The possibility of obtaining sick leave online is a real help in everyday functioning, especially for parents, lonely remote workers, entrepreneurs, or people living in smaller towns where immediate access to a doctor is more a luxury than the norm.<\/p>\n

And although the system is not without flaws, as like any system it is based on trust and professionalism, it is hard to imagine today a return to times when illness meant having to stand for hours in crowded clinics. But it also offers something more: the feeling that the medical system can be truly humane, accessible, and modern. After all, each of us has the right to be sick in peace.
